JLT PERFORMANCE GYM LLC – CLIENT WAIVER AND AGREEMENT

Congratulations on taking the next step in your fitness journey. JLT Performance Gym is committed to helping you reach your goals with expert guidance, effective programming, and a supportive environment.

Health Acknowledgment and Assumption of Risk
I understand that participation in fitness training, whether supervised or unsupervised, involves inherent risks including, but not limited to, cardiovascular stress, muscular strain, sprains, and other physical injuries. I affirm that I am physically able to engage in this program and have either had a physical examination or voluntarily assume the risk of participation. I acknowledge that it is my responsibility to inform the trainer of any medical conditions or injuries.

I voluntarily assume all risks associated with participation in training activities conducted by JLT Performance Gym LLC, including sessions held in-studio, outdoors, or at any off-site or remote location.

Release of Liability
I hereby release and hold harmless Jamal Liggin, JLT Performance Gym LLC, its employees, independent contractors, agents, and assigns from any and all liability, claims, demands, causes of action, or damages arising out of or in connection with my participation in training sessions or related activities, except in the case of gross negligence or willful misconduct. This release is binding upon my heirs, legal representatives, and assigns.

Program Guidelines & Session Policies

  • I understand that results are not guaranteed and are dependent on my effort and consistency inside and outside of training sessions.

  • Personal training sessions range from 50 to 90 minutes. If I arrive late, my session may be shortened accordingly. If my trainer is late, I will receive the full session time.

  • All training sessions are non-refundable and non-transferable. Sessions must be used within 12 months of purchase unless otherwise agreed in writing.

  • All training packages are billed on a pre-pay basis. Payment must be made within the first week of training or prior to the first session.

Accountability & Attendance Policy

  • I understand that purchasing a monthly training package reserves a specific training time. This is a commitment, not a per-session agreement.

  • If I cancel with more than 48 hours’ notice, no penalty will apply.

  • Cancellations made within 48–24 hours may result in a forfeited session, at the discretion of my trainer.

  • Cancellations made less than 24 hours before a scheduled session or no-shows will result in loss of that session.

  • JLT Performance Gym LLC is not responsible for rescheduling missed sessions. Any rescheduling is between the trainer and the client.

Media Release
I hereby grant permission to JLT Performance Gym LLC to use photos, video, and/or testimonials of me captured during training for marketing, promotions, and educational purposes.

Trainer Contact & Touch Policy
During sessions, trainers may use tactile cueing (touch training) for alignment correction and performance enhancement. If I prefer not to be touched or experience discomfort at any time, I will inform my trainer immediately.
